Standard Conditions
Blinded
Can't see and auto-fails ability checks requiring sight. Attack rolls against you have Advantage, and your attack rolls have Disadvantage.
Charmed
Can't attack the charmer or target them with harmful abilities or magical effects. The charmer has Advantage on ability checks to interact socially with you.
Deafened
Can't hear and auto-fails ability checks requiring hearing.
Frightened
Disadvantage on ability checks and attack rolls while the source of fear is within line of sight. Can't willingly move closer to the source of fear.
Grappled
Speed is 0 and can't increase. Disadvantage on attack rolls against any target other than the grappler. The grappler can drag or carry you when it moves, at half speed unless you are Tiny or two or more sizes smaller.
Incapacitated
Can't take actions, Bonus Actions, or Reactions. Can't concentrate on spells. Speech is limited to faltering utterances. Automatically surprised if not already in combat.
Invisible
Impossible to see without magic or a special sense; fully concealed. Attack rolls against you have Disadvantage, and your attack rolls have Advantage.
Paralyzed
Incapacitated, can't move or speak. Auto-fails STR and DEX saves. Attack rolls against you have Advantage. Any attack that hits is a Critical Hit if the attacker is within 5 feet.
Petrified
Transformed into a solid inanimate substance (usually stone). Weight increases by a factor of ten. Incapacitated, can't move or speak, unaware of surroundings. Resistance to all damage. Immune to Poison and disease.
Poisoned
Disadvantage on attack rolls and ability checks.
Prone
Can only crawl (every foot of movement costs 1 extra foot) unless you stand up. Disadvantage on attack rolls. An attack roll against you has Advantage if the attacker is within 5 feet, otherwise Disadvantage.
Restrained
Speed is 0 and can't increase. Attack rolls against you have Advantage. Your attack rolls have Disadvantage. Disadvantage on DEX saves.
Stunned
Incapacitated, can't move, and can speak only falteringly. Auto-fails STR and DEX saves. Attack rolls against you have Advantage.
Unconscious
Incapacitated, can't move or speak, unaware of surroundings. Drops what it's holding and falls Prone. Auto-fails STR and DEX saves. Attack rolls against have Advantage. Hits within 5 feet are Critical Hits.

Exhaustion 2
D20 Tests -4, Speed -10 ft. (Cumulative: levels stack. Long Rest removes 1 level.)

D20 Tests -6, Speed -15 ft. (Cumulative: levels stack. Long Rest removes 1 level.)
Exhaustion 4
D20 Tests -8, Speed -20 ft. (Cumulative: levels stack. Long Rest removes 1 level.)
Exhaustion 5
D20 Tests -10, Speed -25 ft. (Cumulative: levels stack. Long Rest removes 1 level.)
Exhaustion 6
Death. (D20 Tests -12, Speed -30 ft.) You die if you reach this level.
